Patient Release Form (required)

In order for us to be able to treat you in our clinic, Boise Spine Center, you are required to fill sign a release form.  This form has several sections:

1) Privacy Policy

2) Consent to Treat

3) Consent to interpret X-Rays

4) Assignment of benefits (release of records)

5) Financial Policy
